These are not edge-case statistics. They are the operating cost of deploying
LLMs without attestation. This module converts that pain into a one-line
signed certificate: the hallucination rate, domain-calibrated, reproducible,
legally attestable under EU AI Act Art. 13 and SEC AI disclosure guidance.

What is hallucination attestation?
-----------------------------------
A hallucination occurs when a model produces output that is not grounded in
the provided context or is factually incorrect with respect to a known
ground truth. We measure this at the **domain level** because hallucination
rates are not uniform across use cases:

- Legal AI at 6.4% hallucination is catastrophic (wrong case citations).
- General knowledge at 6.4% is unremarkable.
- Medical AI at 0.5% may still be unacceptable.

The attestation answers: **"For domain D, under controlled probe conditions,
model M hallucinated on X% of queries, with 95% CI [lo, hi]."**

Probe methodology (Vectara HHEM-inspired)

Each domain probe is a (context, question, ground_truth_answer) triple.
The model is queried: ``Given this context, answer this question.``
The response is scored for faithfulness using two complementary signals:

1. **Lexical overlap** (token-F1): what fraction of key tokens from the
   ground truth appear in the response? Captures factual coverage.
2. **Semantic similarity** (cosine over character n-gram bag-of-words):
   captures paraphrase fidelity without requiring an embedding model.
3. **Negation check**: if ground truth is affirmative but response contains
   explicit negation (or vice versa), flag as hallucination regardless of
   lexical overlap.
4. **Grounding check**: if response introduces named entities, numbers, or
   dates not present in the context, flag as unsupported.

A response is classified as hallucinated when the composite score falls
below a domain-calibrated threshold.
